International Project Forms
Please feel free to fill out the volunteer team sign up form if you are interested in sending a volunteer team to Puerto Rico. In filling out this form you will be added to our calendar and receive a project cost estimate. This is not a binding document. You are able to make changes to your dates or volunteer numbers, or cancel the trip, up until 30 days before the project start date at no cost to you.
Once your trip has been confirmed and tickets have been purchased your team leader will need to fill out the pre-project information sheet which will provide our team with the information needed to send you a completed project plan for your upcoming trip. In addition, a team leader roster and flight information sheet will also need to be filled out before the project start date. These forms need to be filled out no later than 30 days before the start of your trip.
All volunteers participating in the project will need to fill out the volunteer agreement.
